If you’re filing a claim, we know that you’re already frustrated, and so we want to make the claims process as simple as possible. We care about our Volunteer Card members and want to do all that we can to help you.
To file a claim, simply email email@example.com or call 952.886.7663. A trained staff member will be more than willing to walk you through the process.
Please have ready the following information:
- Volunteer Card number (begins with “V” and is found on the front of your card)
- Date of birth
- Travel dates and destination
- Date incident occurred / you were treated
- Brief description of incident / what you were seen for
- Amount claiming (in dollars)
You will receive an email within 24-48 business hours with a claim number and further instructions, including what documentation is needed to complete the claim.
